We are accepting new patients and would love to care for you!

You can complete a brief enrollment form by clicking the link to sign up below and we will establish care for you on a first-come, first-served basis. All care through Antioch Med comes through our affordable monthly membership.

You will not be charged anything until you have your first appointment.

Why A Membership?

Antioch Med has reenvisioned the way primary care is delivered allowing us to give personalized care and a life-giving patient experience. Why does this require a membership?

Our Direct Primary Care (DPC) membership simplifies and prioritizes the doctor-patient relationship by removing unnecessary third parties that make usual primary care expensive, inaccessible, and fragmented. This allows our family doctors to practice medicine the way it was intended - for the patient's benefit. DPC is a return to the good old days of medicine when doctors knew the whole family and counted patients as friends and not just a number or a diagnosis code.

We offer an affordable monthly membership for care and do not accept any form of insurance for payment. Membership based care is new or different from what many of experienced and many common questions are answered on our FAQ page. Insurance or a medical cost share may still be needed for catastrophic or unexpected events (and we may be able to help you build a more comprehensive solution!). If you have insurance, we can help you to use it wisely for covered services. If uninsured, we will give you excellent primary care all the same!

Membership Pricing

Antioch Med Memberships provides access to your doctor and numerous services through an affordable monthly membership. Membership pricing is based on age and not determined by medical complexity. 

Individual Members

  • Child (Age 0-17) - $49/month

  • Adult (18+) - $79/month

    (family cap of $250/month)
